To the Editor: Although the “three-step test” is regarded as the gold standard for diagnosing fourth nerve palsies, there are a number of instances in which its usefulness is diminished, including restrictive ocular myopathies, longstanding strabismus, strabismus surgery, skew deviation, and disorders such as myasthenia where more than one muscle is involved.1 Jacobson et al. used the

We keep it nice and simple! If you like what we do, The trochlear nerve is the fourth cranial nerve and is the motor nerve of the superior oblique muscle of the eye. It can be divided into four parts: nucleus and an intraparenchymal portion cisternal portion cavernous sinus portion orbital Trochlear nerve palsy is mentioned in ophthalmology texts dating to the mid nineteenth century. However, it received little more than a brief mention and was no doubt an underrecognized entity. In 1935, Bielschowsky correctly noted that trochlear nerve palsy was the most common cause of vertical diplopia and introduced his classic head-tilt test. Carter N, Miller NR. Although trochlear nerve palsy accounts for a majority of cases of acquired vertical strabismus, trochlear nerve palsies are less commonly recognized than oculomotor or abducens nerve palsies. A lesion involving the trochlear nucleus or its fascicles may result in contralateral paresis of the superior oblique muscle.
